Your first session with a Health Coordinator will help them understand the support you need. They’ll ask questions about your history, current situation, and factors like your work, family, housing, and mental and physical health. As sessions progress, your coordinator will partner with you to explore solutions and pathways that suit your individual circumstances.

Our Health Coordinators offer personalised support through one-on-one sessions and group sessions. They are skilled listeners, helping clients overcome challenges and barriers while providing the option of face-to-face or telehealth appointments to ensure the highest quality care.
