An EPC is a treatment plan for a chronic condition that gives you access to five sessions with an allied health professional in a calendar year who has been recommended by your GP. These sessions are subsidised by Medicare.
The Chronic Disease Management (CDM) Medicare categories are for General Practitioners (GPs) to manage patients with chronic or terminal medical illnesses, particularly those requiring multidisciplinary, team-based care from a GP and at least two other health or care professionals.
Services for Chronic Disease Management (CDM) assist qualified practitioners in coordinating patient care for those with terminal or chronic illnesses.
